Instrucciones previas: conocimientos, actitudes y valoración de los profesionales de enfermería (Estudio descriptivo)

Abstract

Las Instrucciones Previas hacen referencia al documento en el que un individuo explicita las instrucciones que deberán tenerse en cuenta cuando su estado de salud no le permita expresarlas él mismo, especialmente en lo relativo a tratamientos médicos y al posterior destino de su cuerpo y órganos. En España están reguladas por la Ley 41/2002, básica reguladora de la autonomía del paciente y de derechos y obligaciones en materia de información y documentación clínica y en Castilla y León por la Ley 8/2003, de 8 de abril sobre los derechos y deberes de las personas en relación con la salud. Los objetivos de este estudio son determinar los conocimientos, las actitudes y la valoración que tiene el personal de enfermería sobre las Instrucciones Previas. Se realiza un estudio descriptivo, observacional y transversal, llevado a cabo a través de una encuesta realizada a 30 profesionales de Enfermería del Hospital de León, que trabajan en la Unidad Coronaria y en la planta de Cardiología. Los resultados obtenidos se han analizado estadísticamente mediante el programa EpiDat 4.1. Se deduce que la mayoría de los encuestados saben de la existencia del Documento de Instrucciones Previas, pero consideran que sus conocimientos sobre el mismo son escasos. No poseen el hábito de consultar el Registro de Instrucciones Previas, porque la mayoría no sabe cómo hacerlo, ni de preguntar a los pacientes si lo tienen formalizado. Las enfermeras, valoran muy positivamente el Documento de Instrucciones Previas, tanto para su entorno más cercano, como para los enfermos, pero ninguna reconoce haberlo realizado como usuaria. Sería conveniente realizar acciones formativas entre los profesionales de enfermería, para adquirir los conocimientos necesarios, informar a los pacientes y ser garantes del cumplimiento del Documento de Instrucciones Previas.

The Previous Instructions refer to the document in which an individual explains the instructions that should be taken into account when his state of health does not allow him to express them himself, especially regarding medical treatments and the subsequent destination of his body and organs. In Spain they are regulated by Law 41/2002, basic regulation of the autonomy of the patient and of rights and obligations in the matter of information and clinical documentation and in Castile and Leon by Law 8/2003, of 8 April on the rights and Duties of persons in relation to health. The objectives of this study are to determine the knowledge, attitudes and assessment of nurses on the Prior Instructions. A descriptive, observational and cross - sectional study was carried out through a survey of 30 Nursing professionals of the Hospital de León, who work in the Coronary Unit and in the Cardiology department. The results obtained were statistically analyzed using the EpiDat 4.1 program. It follows that the majority of respondents know of the existence of the Document of Preliminary Instructions, but consider that their knowledge about it is scarce. They do not have the habit of consulting the Register of Preliminary Instructions, because most do not know how to do it, nor to ask patients if they have it formalized. Nurses highly value the Pre-Procedure Document, both for their immediate surroundings and for the sick, but none recognizes having performed as a user. It would be advisable to carry out training actions among the nursing professionals, to acquire the necessary knowledge, inform the patients and be guarantors of the fulfillment of the Document of Previous Instructions.
