Aseguramiento de la calidad de un ecógrafo para braquiterapia. Revisión del estado de referencia inicial

Abstract

La braquiterapia prostática es un tratamiento del cáncer de próstata que introduce fuentes radiactivas en el paciente para irradiar al tumor desde dentro con el objetivo de reducirlo de tamaño y tratar de eliminarlo. Este procedimiento se realiza guiado por ecógrafo, para tener imagen en vivo de dónde se depositan estas fuentes y adaptar el tratamiento si fuera necesario. El papel del ecógrafo en esta intervención es esencial, y su correcto funcionamiento debe ser verificado. Esto es labor de los radiofísicos, tanto la verificación del estado de calidad como la elaboración del estado de referencia inicial. Este trabajo de fin de grado está orientado a asegurar el estado de calidad del ecógrafo utilizado para braquiterapia en el Hospital Clínico Universitario de Valladolid, y a partir de las pruebas de comprobación elaborar el estado de referencia para futuras verificaciones. Para ello, se van a llevar a cabo pruebas de resolución, medidas de distancias en distintos ejes, comprobaciones de volúmenes, entre otras, obtenidas de distintos protocolos de asociaciones de Física Médica. Con los resultados obtenidos se van a valorar las capacidades del ecógrafo y las consecuencias que sus limitaciones puedan causar en el procedimiento de braquiterapia de próstata, determinando si es apto para su uso. Además, se va a elaborar el estado de referencia inicial, cuyo papel consiste en facilitar los controles de calidad de los siguientes años, ya que se tienen datos concretos de las medidas del ecógrafo para esas mismas pruebas y facilitará la detección de errores en su funcionamiento. Finalmente, se va a concluir que el ecógrafo empleado es aceptable, y las medidas que se salen de las tolerancias dadas por los expertos serán todas justificables por diversas causas ajenas al funcionamiento del aparato

Prostate brachytherapy is a prostate cancer treatment which introduces radioactive sources inside the patient to irradiate the tumor from within, in order to reduce its size and try to eliminate it. This procedure is performed guided by ultrasound, to have a live image of where these sources are implanted and adapt the treatment plan if needed. The role of the ultrasound system in this intervention is key, and its correct performance must be verified. This is part of the job of the radiophysicists, both the quality assurance and the development the initial reference status. This final degree project is aimed at ensuring the quality status of the ultrasound system used for brachytherapy at Hospital Clínico Universitario de Valladolid and, based on the assurance tests, developing the reference status for future verifications. To achieve this, several tests will be carried out, such as resolution tests, distance measurements on different axes, volume checks, among others, obtained form different protocols of Medical Physics associations. With the obtained results, the capabilities of the ultrasound system and the consequences that its limitations may cause in the prostate brachytherapy procedure will be assesed, determining whether it is suitable for use. In addition, the initial reference state will be developed, in order to facilitate the following years’ quality assurance, since there are data on the ultrasound measurements for these same tests and it will make the detection of errors in its functioning easier. Finally, it will be concluded that the ultrasound system used is aceptable, and the out of tolerance measurements will all be justifiable for several reasons unrelated to the operation of the device
