Protocolo de acompañamiento a pacientes después del alta hospitalaria en SACYL: una propuesta desde el trabajo social sanitario

Este Trabajo Fin de Grado aborda, desde una revisión bibliográfica, la complejidad del
denominado como proceso de recuperación de los/as pacientes tras el alta hospitalaria. Esta etapa
muestra un momento crítico dentro del proceso de recuperación ya que la tendencia a la reducción
de las estancias hospitalarias, junto con la falta de apoyos tras la salida del hospital genera una
serie de factores de riesgos con graves consecuencias: emocionales, sociales y familiares.
Dentro del ámbito hospitalario, el alta médica no marca el final del proceso asistencial sino
el comienzo de una fase especialmente delicada para numerosos pacientes. La falta de un protocolo
estandarizado deja a muchos/as pacientes y a sus familias en circunstancias de vulnerabilidad y
desprotección.
Ante esta realidad y como propuesta de valor, se ha diseñado un Protocolo de
Acompañamiento post-alta hospitalaria desde la perspectiva del trabajo social sanitario, centrado
en el sistema público de salud de Castilla y León (SACYL). Esta propuesta busca garantizar una
transición adecuada y estructurada del hospital al hogar, con el fin de evitar nuevas
complicaciones, reducir los reingresos y contribuir con la mejora de calidad de vida de los/as
pacientes. A través de esta iniciativa, se pretende no desatender al/la paciente una vez salga del
centro hospitalario, fortaleciendo así la red de apoyo social y mejorando la calidad de vida en su
proceso de recuperación.
This Final Degree Project addresses, through a literature review, the complexity of the so-
called recovery process of patients after hospital discharge. This stage represents a critical moment
in the recovery process, as the trend towards shorter hospital stays, together with the lack of support
after leaving hospital, generates a series of risk factors with serious consequences: emotional,
social and family.
Within the hospital setting, medical discharge does not mark the end of the care process
but rather the beginning of a particularly delicate phase for many patients. The lack of a
standardised protocol leaves many patients and their families in vulnerable and unprotected
circumstances.
In view of this reality and as a value proposition, a Post-Hospital Discharge Support
Protocol has been designed from the perspective of healthcare social work, focusing on the public
health system of Castile and León (SACYL). This proposal seeks to ensure an adequate and
structured transition from hospital to home, with the aim of avoiding new complications, reducing
readmissions and contributing to improving patients' quality of life. Through this initiative, the
aim is to ensure that patients are not neglected once they leave the hospital, thus strengthening the
social support network and improving their quality of life during their recovery process.
