Escuela de Doctorado K1 Estrés K1 Professional stress K1 Estrés profesional K1 Psychological stress K1 Estrés psicológico K1 Emergency K1 Urgencias K1 2411-Fisiología Humana AB There is no doubt about the relevance of the emergency departments of Spanish hospitals as first-line health care providers, mainly attending to emergencies.The health professionals working in them must cope with environmental demands that frequently exceed the individual's response capacities, leading to high stress levels. The aim of this thesis is to carry out a biopsychological assessment of health professionals in the hospital emergency departments of the Hospital Santa Bárbara de Soria (HSBS) and Hospital Clínico Universitario de Valladolid (HCUV), evaluating physiological hormonal markers (cortisol, α-amylase and dehydroepiandrosterone in saliva) to study the possible influence of the pattern of these biomarkers on the anxiety, self-efficacy and sleep quality of these professionals. This research is complemented by assessing the professional quality of life of the health professionals involved.For this purpose, three analytical, descriptive and cross-sectional studies were carried out in the HSBS and HCUV, two public hospitals of the Castilian-Leonese health system. A total participation of 97 professionals was obtained, 59 were nursing professionals (10 men and 49 women) and 38 were medical professionals (10 men and 28 women) of whom 45 professionals worked in the HCUV and 52 in the HSBS. Four different times of the day were chosen to obtain salivary samples in order to assess the circadian rhythm of secretion (8:00h, 12:00h, 15:00h and 00:00h/24h). They were also asked to answer the State-Trait Anxiety Inventory (STAI; State-Trait Anxiety Inventory), Oviedo Sleep Questionnaire (COS), Professional Quality of Life (CVP-35), Baessler and Schwarzer General Self-Efficacy Scale, Medical Staff Stress Survey (MPSS-R) and a sociodemographic questionnaire created ad hoc for the research.The results showed decreasing cortisol levels during the working day, with maximum values at 8:00h and minimum at 24h. A similar pattern was observed for DHEA. Levels of α-amylase (a reflection of the sympathetic-adreno-medullary response) increased throughout the working day, reaching its maximum value at 15:00h, and decreasing at 24h, compared to the data for the rest of the working day.In relation to psychological variables, self-efficacy, anxiety and sleep quality scores remained in normal ranges. This is supported by the modest correlation between STAI State-Trait scores and salivary levels of cortisol, α-amylase and dehydroepiandrosterone.Regarding the professional quality of life, the responses to the questionnaire indicate that the studied participants have a good perception of professional quality of life. This is reflected in the three dimensions of the questionnaire: managerial support, workload and intrinsic motivation.In conclusion, stress is a present fact among emergency department medical and nursing professionals. On the other hand, the increase of DHEA, due to its anabolic condition, could counteract and modulate its effect, suggesting a positive effect on their professional performance. In this context, despite being a study conducted prior to the COVID-19 pandemic, the emergency professionals of the hospitals studied seem to have a proportionate management of the work. Overall, it means that the stress generated during work performance is controlled, allowing a correct adaptation to the demanding situations experienced in the emergency departments.
