Most of the time, sepsis may be complicated by organ dysfunction giving rise to the most severe form: septic shock. This last form could present great clinical similarities to a shock due to other etiology, without any possibility to determine the septic nature facing the same clinical symptoms in our patients. Additionally, shock is recognized as the main death cause in the Intensive Care Units worldwide. Several studies had tried to contribute to a better sepsis diagnosis, through the analysis of the pathophysiology and the immunologic changes at hosts. Whilst, shock molecular mechanisms still represent a huge challenge for our medical services. Therefore, the first aim of our study is to analyze the presence of specific genes expressed in patients with septic shock against shock patients from other etiology, in order to identify specific biomarkers for the first ones. Moreover, it will attempt to assess the survival probability of shock patients with another shock origin. Nowadays, different scales are used for this purpose, such as APACHE II or SOFA. These systems are based on clinical data, taking no notice of the molecular changes. Besides, clinical decisions are strongly joined to the ICU physicians’ criteria. Hence the development of new clinical tests, which evaluate the molecular changes through the transcription standard analysis, could mean a qualitative leap in mortality prediction in these patients. Thus, our second goal is to identify the gene expression patterns for stratifying patients with postsurgical shock in: high or low mortality risk.
