While there is consensus on heart failure and uncontrolled infection being indications of cardiac surgery in this disease, scientific evidence supporting surgery just to avoid embolic events is scarce. In the present doctoral thesis, we present two works showing the limitations of prevention of embolic events as indication of surgery. We add a narrative review of embolic events in infective endocarditis. The first article is a retrospective study that suggests that cardiac surgery just to prevent embolic events patients with large vegetations without heart failure or uncontrolled infection is not beneficial. In the second article we analyze the interobserver variability between two experiment observers in the measurement of vegetation size by echocardiogram. This study concludes that interobserver variability is high and may change the surgical indications frequently. The third article is a narrative review about embolic events in infective endocarditis.
